IDV Code Request Form Set Up

Introduction

When working in your system, setting up an IDV (Identity Verification) request form ensures you can quickly collect and manage the necessary client identification data through your client portal. This guide walks you step-by-step through accessing, customising, and saving your IDV form so it’s ready to send as part of your client information requests.

IDV Code Request Form

1: Access the Client Portal Form Builder

  1. Go to the Admin section of your system.
  2. Scroll down to find Client Portal Form Builder.

2: Locate the IDV Form

  1. In the Form Builder, select View Panels.
  2. From the panel list, choose IDV Form.
  3. Click View on the IDV Code Request panel to load the form.

 3: Review and Configure Panel Details

  1. Panel Name – Verify it’s correct.
  2. Involvement Type – Usually set from organisation to people.
  3. Organisation Scope
    • Toggle whether the request applies to all organisations linked to the recipient.
    • The recipient can be Primary Person, Finance Person, or Admin Person.
    • By default, it’s set to Admin Person.

 4: Select Recipient Type

  1. Use the dropdown to choose who will receive the request.
  2. Adjust as needed based on your internal process (Primary, Finance, or Admin).

 5: Review Included Fields

  1. Default fields: Name, Organisation, IDV Code, Email Address, Mobile Number.
  2. Note:
    • Name and Organisation are read-only.
    • IDV Code is mandatory.
  3. If needed, add more fields by selecting them from the field list.

6: Set Auto-Populate Conditions

  1. By default, the form includes anyone who is a Director, PSC (Person of Significant Control), or Secretary of the organisation.
  2. You can choose to include ceased involvements (people who have left the business) by ticking the relevant box.
  3. Add extra conditions if necessary (e.g., using involvement type from integrated systems like CCH).

7: Save Your Form

  1. After making adjustments, click Save.
  2. Your updated IDV request form is now ready to be used in client information requests.
